211 Programs.2-1-1 is an easy to remember telephone number that connects callers to information about critical health and human services available in their community. Dial 2-1-1 24 hours a day and 7 days a week to speak to an Information & Referral Specialist.
Health Care Program for Children with Special Needs (HCP). The Health Care Program for Children with Special Needs helps the families of children with, or at risk for, such conditions as asthma, autism, cancer, cerebral palsy, cleft palate, cystic fibrosis, developmental disabilities, diabetes, kidney disease, loss of hearing or sight, sickle cell anemia, TBI and more. Anyone can refer a child to HCP. Please call 303-239-7006 or 303-582-5803.
Nurse Family Partnership Information: The Nurse Family Partnership Program is for first-time moms, who meet low-income requirements and live within the service area. You are encouraged to enroll as early as possible in your pregnancy. A caring registered nurse will visit you in your home throughout your pregnancy and up until your baby is 2 years old - at no charge. Call 303-809-6395 or 303-582-5803 for more information. First Time Moms Pregnancy Assistance

WIC Program. WIC provides nutritious foods, nutrition education (including breastfeeding promotion and support), and referrals to health and other social services to participants at no charge. WIC serves low-income pregnant, postpartum and breastfeeding women, as well as infants and children up to age 5. WIC participants receive monthly checks for helathy foods to supplement their diet. Apply for WIc by calling 303-670-7546 or 303-582-5803.

Cardiovascular Testing. Gilpin County Public Health Agency provides a FREE: Total Cholesterol, LDL, HDL, Triglyceride, Blood Glucose (Diabetes check), Blood Pressure, Oxygen testing, Body Mass index testing and Peripheral Arterial Disease Testing. Other cardiovascular testing is also available at low cost: A1C test - $10; Nitric Oxide Testing $5. Call 303-582-5803 to set up an appointment at 101 Norton Drive, Black Hawk, CO.
